The Form feature is exclusive for Workflow Plus users, and you can upgrade in your settings.

You choose to send it directly to a client, or you can create a generic one if you want to send it to new clients.

You'll find it inside the Client's dashboard > Forms

You can add as many questions as you want and choose if the answer should be a text, a single choice, or multiple choices.

You'll type the question on the box, and add the items if it's not a text answer. 

You can also reorder the questions by clicking the arrows on the right side, or remove a question on the red "x".

If you feel that it's time to finish and send your questionnaire, click on the "finish editing" button, and our system will generate a URL for you to share, or you can click to send it by email.
